The documents we require from families when enrolling their child into our service include:
- Birth Certificate
- Immunisation History
- Allergy Action Plan (if child has an allergy)
- Asthma Action Plan (if child has asthma)
- Anaphylaxis Action Plan (if child has anaphylaxis)
- Diabetes Action Plan (if child has diabetes)
- Copy of Medicare Card
- Copy of Centrelink Card (if applicable)
